Failed targeted read test

I have gotten a failed targeted read test.

I did F12 Diagnostics scan

I received :

ERROR CODE 2000-0142

Validation 109536

MSG:  Hard Drive 0- S/N  S2WKWB4P,  Short self test unsuccessful

I am running a Dell Inspiron 3520 that I got on 12/25/2012

I have been having problems for a few months with the Tiles not working.  I am unable to open anything from the tiles.

Please advice on what I need to do.
